Certified Home Health Aide : Available to work Monday through Friday, 8:30am to 4:30 pm, to provide quality end of life care, including personal care and companionship to our patients. Must possess a Home Health Aide Certificate, Drivers license and have a vehicle in good working order. $20 per hr + benefits
Social Work Per Diem (LCSW) : Hospice Social Work per diem services are based on an initial and ongoing assessment of patient and family needs by a Social Worker from an accredited school of Social Work, and in accordance with state/federal regulations. Licensed Clinical Social Worker.Visits patients at home and in facilities. Hospice experience preferred. $80 per visit
